The tariff matters too: under Ukraine's division of authorities each document type has its own fee (education - the Ministry of Education, notarial and court documents - the Ministry of Justice, criminal record - the Ministry of Internal Affairs). For a state-issued medical certificate the sequence is: first, pre-certification at the regional health department - confirming the medical institution's signature and seal - then submission for the apostille at the MFA in Kyiv.
